Forms detailing your medical history are an important component to providing the right treatment
If you can, we encourage you to fill out the necessary forms prior to arriving at the hospital. This can shorten your wait time. However, we understand that it's not always possible to fill them out ahead of time so if you do need to wait to fill them out at the hospital, please be sure to arrive early for your appointment.
Forms will document your personal medical history as well as your family's. They are essentially a history of you up until this moment and will help us in the present as well as in the future.
