Our intensive diabetes management program is helping high-risk patients improve blood sugar, control blood pressure and avoid life-changing complications

The article follows patients and care teams across MLKCH and MLK Community Medical Group, showing how coordinated support — including primary care, endocrinology, pharmacy, nutrition education, community health workers and Recipes for Health — is helping patients better manage diabetes and avoid serious complications.
Four years after launching the intensive management program, more than 80% of enrolled patients have seen blood sugar levels decline, more than 70% have brought their blood pressure under control, and diabetic-related amputations among program patients have dropped to virtually zero.
